IMM RN Medical Case Manager

The RN Care Case Manager (RN RW IMM) blends the mission with Care Management across both Ryan White and Pharmacy programs. Utilizing strong nursing clinical skills, RW IMM Nurse focuses on a caring and knowledgeable approach. You’ll work with our providers, pharmacy and healthcare center team so that patients receive the support they require and access to available resources.

The primary responsibilities of this role will include but is not limited to providing leadership to the healthcare center staff with regards to healthcare plan quality improvement by continuous measuring and monitoring the improvement of our healthcare center operations.
